  12. direct that my life shall not be artificially prolonged under the

  20.     a. I have an injury, disease, or illness which is not
  21. curable or reversible and which, in their judgment, is a
  22. terminal condition; and

  26. I have been unconscious, comatose, or otherwise incompetent
  27. so as to be unable to make or communicate responsive decisions
  28. concerning my person; then,

  31.     I direct that life-sustaining procedures shall be withdrawn
  32. and withheld, it being understood that life-sustaining procedures
  33. shall not include any medical procedure or intervention for
  34. nourishment or considered necessary by the attending physician
  35. to provide comfort or alleviate pain.
